CMCs are one-stop centres that provide a wide range of comprehensive services for patients with chronic diseases. The majority of patients are referred by GPs or some can be walk-in cases.

The services provided by CMCs include:

• Coordinating and co-managing with GPs to manage their patients with chronic diseases.
• Regular face-to-face and telephonic follow-up. This is to encourage patients to adhere to the treatment regimes and lifestyle modifications recommended by their GPs.
• Patient education. Patients are educated and trained to use tools such as glucometers (blood sugar self-tests), asthma inhalers and techniques such as self-administering insulin injection and blood pressure taking.
• Diabetic Retinal Photography (DRP). Patients with diabetes are required to do retinal examinations at least once a year to monitor for eye complications.
• Podiatry. Patients with diabetes are required to go for annual foot screening to detect any foot complications which may lead to amputation. Follow-up podiatric services are available to patients as well if necessary.
• Medication counselling.
• Empowerment workshops on chronic diseases for patients.
• Organisation of support group meetings.
